Latest Multibreed Title Award Request


Please complete the following form to request your most recent Multibreed Award for an Inactive/Retired/Deceased dog. Previous awards are not available at this time. You do not need to make a request for an Active dog as these awards will automatically be issued.

Your club's captain and/or owner have the ability to request pins/certificates on behalf of club members all at once through another online form, (which may be easier in the case where you need to request for multiple dogs). However, plaques must be requested by dog owners (MB ONYX, MBGDCH) through this form.

Delivery times for these awards is dependent on the level of requests, and delivery of regular awards and multibreed awards for Active dogs has priority. If you have questions please contact awards@flyball.org.
