If your dog is on the Inactive, Retired, Deceased Iron Dog Award Recipient List and you wish to receive a plaque:

Fill in the special requests form. Please note: The December 18th, 2011 deadline for ordering IRON DOG plaques for delivery in early 2012 has passed. You may still order a plaque, but the delivery date will be based on receiving enough further orders to warrant an additional plaque order before the end of this fiscal year. Questions may be directed to awards@flyball.org.

With this same form, you may request a correction to the name, and specify how you would like to receive the plaque (Regional Director or direct delivery).

If your dog is on the Active IRON DOG Award Recipient List your plaque will be sent directly to your Regional Director. No further action is required.
